Terms and conditions
These terms apply to enrollment in any Kodama program: tinnitus, hyperacusis, or misophonia. They work alongside, not instead of, the Informed Consent and Service Agreement specific to your program, which you review separately in Practice Better.

Eligibility and enrollment
Kodama's programs are for adults 18 and older. There are no state-specific restrictions on where you live. Enrollment is subject to Kodama's eligibility review; purchasing or selecting a program does not by itself guarantee acceptance. Kodama may delay, modify, or decline enrollment when additional medical, audiological, psychiatric, psychological, or other clinical evaluation is needed.
Program fee and payment
The program fee shown at checkout in Practice Better is the price that applies to your enrollment, including any founding-participant rate already reflected there. You may pay in full at enrollment, or use an interest-free payment plan of three automatic payments over sixty days. The payment plan does not increase the program price and carries no interest or finance charge. By selecting it, you authorize Practice Better's payment processor to charge the payment method on file on the dates shown at checkout, and you'll receive a receipt for each payment. The payment plan divides the timing of payment only; it is not a subscription and does not renew after the third payment.
The founding-participant rate is offered in exchange for agreeing to complete three brief founder feedback surveys about your experience with the program: one at the start, one partway through, and one at the end. These ask about the program itself, not your clinical care, and are separate from any research participation, which remains its own separate, optional consent (see the privacy policy). Not completing a founder feedback survey does not affect your enrollment, access, or the price already applied to your account.
If a scheduled payment fails, Kodama will notify you so that payment arrangements can be resolved. Modules are released on the program’s fixed schedule and are not withheld from an individual participant. If payment remains unresolved, Kodama may end enrollment as described under cancellation and refunds, with any earned balance still due.
How your program fee is allocated
Your program fee is earned in stages as you're given access to each part of the program. If you cancel after access has begun, the refund due follows the allocation below for your specific program.
Hyperacusis Program
10% is allocated to eligibility review, enrollment setup, baseline review, and release of Getting Ready; 9% is allocated to each of Modules 1 through 10. At the founding rate of $400, that's $40 for Getting Ready and $36 for each numbered module.
Misophonia Program
12% is allocated to eligibility review, enrollment setup, baseline review, and release of Getting Ready; 11% is allocated to each of Modules 1 through 8. At the founding rate of $300, that's $36 for Getting Ready and $33 for each numbered module.
Tinnitus Program
The tinnitus program has no separate Getting Ready release, so eligibility review, enrollment setup, and baseline review are included in the allocation for Module 1. 12.5% is allocated to each of Modules 1 through 8. At the founding rate of $300, that's $37.50 per module.
Cancellation and refunds
You may request cancellation through the secure Practice Better portal or by emailing billing@kodamahealth.com. Cancellation does not require a phone call or an explanation.
Before program access begins: if you cancel before any program materials are released to you, Kodama will refund all program payments.
If Kodama determines you are not eligible: if Kodama determines, before program access begins, that the program is not appropriate for you, Kodama will cancel enrollment and refund all program payments.
After program access begins: refunds are based on the portion of the program already delivered, using the allocation above for your program. A portion becomes earned once that component has been completed or released to you. After cancellation, Kodama calculates the earned amount, cancels any future installments, and compares the earned amount with payments already collected. If you've paid more than the earned amount, the difference is refunded; if you've paid less, the remaining earned balance stays due. Approved refunds are submitted to the original payment method within 10 business days; your bank or card issuer may require additional time to post the credit.
Program period and access
The scheduled coach-guided period begins when Kodama accepts enrollment and releases your first program materials. For the hyperacusis program, this guided period is four months; for the tinnitus and misophonia programs, it's three months. Program materials remain available through Practice Better for twelve months after activation, subject to these terms and the continued availability of the platform. Weekly coach guidance and active check-in review are included only during the guided period unless Kodama agrees in writing to an extension.
Before program access begins, enrollment may be deferred when required forms or consent documents are incomplete or when new symptoms require outside evaluation. After access begins, modules are released on the program’s fixed schedule; Kodama does not pause or hold an individual participant’s modules. Kodama may end participation when a safety screen triggers the program’s fixed referral-out response, when your needs exceed the program’s scope, when payment obligations remain unresolved, or when continued participation is no longer appropriate. Refunds then follow the allocation above.
Technology and communication
Participation requires a compatible internet-connected device, an email address, and access to Practice Better. You're responsible for keeping your login credentials secure and for telling Kodama if you believe your account has been accessed without authorization. Messages and check-ins are reviewed regularly, but Practice Better is not continuously monitored and must not be used for emergencies. Kodama is not responsible for temporary interruptions caused by maintenance, internet disruption, device failure, or circumstances outside its reasonable control.
Personal use of materials
Program videos, written materials, worksheets, exercises, and related content are provided solely for your personal use as an enrolled participant. Without written permission from Kodama, you may not share your account or access links, copy or distribute program materials, record or republish program content, or use the materials to provide services to another person.
Respectful use
You agree not to use Practice Better or other program communication channels to harass, threaten, impersonate, or harm another person, or to attempt to obtain unauthorized access. Kodama may restrict or terminate access for material misuse after considering patient safety, continuity-of-care responsibilities, and applicable law. Any refund in that case is determined under the cancellation and refunds section above.
Privacy
How your information is collected and used is described in the privacy policy.
Voluntary withdrawal
You may stop participating at any time. Withdrawal doesn't affect your ability to seek care elsewhere. Financial consequences of withdrawal are governed by the cancellation and refunds section above. Kodama may provide transition or referral recommendations when clinically appropriate.
No guarantee of results
Individual results vary. Purchasing or completing a program does not guarantee symptom improvement or any particular clinical result.
Relationship to your informed consent
You must separately review and sign the Informed Consent and Service Agreement specific to your program. If these terms conflict with the informed consent regarding clinical care or safety, the informed consent and applicable law govern the clinical issue.
Changes to these terms
The terms accepted at enrollment govern your purchase. Kodama may update these terms prospectively, but a material change affecting your existing enrollment won't apply retroactively without appropriate notice.
Applicable law and contact
These terms are offered by Veronica Hollabaugh, LLC, d/b/a Kodama, based in Bay Village, Ohio, and are subject to applicable federal and state law, including any mandatory consumer protections in the jurisdiction where services are provided. By enrolling, you confirm that:
- You have read and understood these terms and conditions.
- You have reviewed and signed your program's Informed Consent and Service Agreement.
- You understand the program fee, payment plan, and refund/cancellation terms above.
- If enrolling at the founding-participant rate, you understand it comes with an invitation to complete three brief founder feedback surveys about the program, not a requirement, and not tied to research participation.
- You understand this program does not replace emergency care.
